Services

Hospice care is available for anyone with a life-threatening illness that has been given a prognosis of six months or less.  Upon admission, Hospice Family Care develops a Plan of Care that addresses physical, as well as the spiritual and emotional needs of the patient and their family.  This includes the following:
  • Medical care for pain and symptom management
  • Comfort and encouragement for family and caregivers
  • A caregiver support group that meets twice per month
  • Assistance with processing paperwork and seeking resources for financial support, as needed
  • Spiritual care, in accordance with the beliefs of the family
  • Guidance and education to prepare the family during the dying process
  • Extensive grief and bereavement services for both adults and children
