Services

Hospice care is available for anyone with a life-threatening illness that has been given a prognosis of six months or less. Upon admission, Hospice Family Care develops a Plan of Care that addresses physical, as well as the spiritual and emotional needs of the patient and their family. This includes the following:
- Medical care for pain and symptom management
- Comfort and encouragement for family and caregivers
- A caregiver support group that meets twice per month
- Assistance with processing paperwork and seeking resources for financial support, as needed
- Spiritual care, in accordance with the beliefs of the family
- Guidance and education to prepare the family during the dying process
- Extensive grief and bereavement services for both adults and children
