• Call our office at (256) 650-1212 and ask to speak with one of our Intake and Admissions Coordinators.  They will need some background information on the patient and will then explain the admission process and let you know how we will proceed on your case.
  
  
• Fill out our  Referral Form and mail it or fax it to our office.

  Hospice Family Care will contact you upon receipt of the completed form.
  
  
• Talk to your doctor.  He/she can discuss the appropriateness of hospice care and call our office to set up an information visit and assessment by our staff.  Please note: if you would like to choose Hospice Family Care, it is very important to specify that to the physician.  There are multiple providers in Madison County, and each organization operates differently.  Sometimes patients think they have our services, when in fact, they've been referred to a "for-profit" group due to miscommunication with a physician.
  
  
• If the patient is in the hospital, speak to the Discharge Planner or Social Services Manager for a referral to Hospice Family Care.  He/she will call our office and arrange for an assessment of the patient in the hospital, or make arrangements for Hospice Family Care to provide services once the patient is back at home.  Again, it is crucial to specify by name which hospice provider you are choosing because there are multiple providers in this community.
