Help for you and your family
Anyone can make a referral for hospice care. Whether it's for yourself, someone in your family or a neighbor or friend... if someone has been diagnosed with a terminal illness and has a predicted life expectancy of six months or less, hospice care may be an option to consider.
There are several criteria for a hospice admission, including:
- Two physicians (patient’s primary physician and hospice medical director) have agreed that hospice care is appropriate and the patient has a predicted life expectancy of six months or less.
- The patient and family want hospice care.
- The patient and family understand that the patient is no longer being treated for a cure.
